This was the first step in the triage system. It would have been placed on the soldier by a stretcher bearer at the advanced aid station. The wound is a GSW gunshot wound, dated 1917 and marked as severe.

To save time officers had a pad with pre-printed requests – the officer simply completed the form as appropriate and sent it back to the company headquarters by runner. In this case the officer is not in touch with the left flank and is being shelled with high explosives. This is dated April 1917
